From Hockey to Clinical Practice

Working with professionals, particularly the Salavat Yulaev hockey team, fundamentally changed how I work in the clinic and my approach to treatment. In sports, the main goal is to get the athlete back to competition quickly and effectively. We used every available resource: from modern physiotherapy techniques to proper pharmaceutical support.

 

The rehabilitation approaches from elite sports transferred easily to the clinic setting. Today I apply exactly the same methods with regular patients. Because the physiology of injury doesn't distinguish whether you're a professional athlete or an office worker who decided to run a marathon. A torn meniscus requires the same level of professional care. The key difference is that in sports, I followed the athlete continuously: I knew their condition before injury, during, and after. I knew their psychological characteristics, how they respond to pain, to exertion. In the clinic, patients come episodically, and we need time to establish that same level of trust.

 

 

Why Regular People Get Injured

I see the same pattern over and over: a mismatch between physical demands and the body's capabilities. People who have never exercised decide on Monday to start a new life – they join a gym, start running, often without a trainer, following videos from the internet.

 

Another problem is our mental attitude toward medicine. Many people think that if they're healthy, they don't need a preliminary medical examination. Getting a clearance certificate to participate in a marathon is seen as a formality. When in reality, it's an opportunity to undergo minimal screening and get a snapshot of your health, to see the "warning flags."

 

In my consultations, I see how the pattern of injuries is changing. If previously we more often saw consequences of heavy physical labor or age-related changes, today the patient profile has become much broader. Musculoskeletal problems now manifest differently: injuries from the sudden surge in fitness popularity dominate, but at the same time many come with chronic conditions caused by sedentary lifestyles or work-related issues.

 

 

 

Diagnosis: Why Treating Only the Local Problem Doesn't Work

People often come to me complaining "my shoulder hurts" or "my knee hurts." In the past, I might have focused only on that area. Now my approach has changed fundamentally. We need to know the patient's baseline condition: hemoglobin levels, whether the body has sufficient nutrients and protein, how all body systems are functioning. Taking a purely local approach and treating just that area is ineffective. Sometimes the shoulder hurts, but the problem is somewhere completely different. When we begin comprehensive assessment, we find weak links: nutritional deficiencies, gastrointestinal problems, iron deficiency states.

 

Patients often "filter" their complaints: "I'll tell you this, but I'll tell another doctor that." As a result, we get separate pages but don't see the whole book. The person goes from doctor to doctor without getting results.

 

To fully understand the causes of pain and dysfunction, and to develop an effective treatment and rehabilitation program, examination alone isn't enough.
